Pines makes it easy to transfer to a new provider either from within Pines or from another health care organization Switching from one provider organization to another can be a challenge, but not at Pines Health Services. Our medical office specialists will be with you every step of the way.

The process is easy:
1) Fill out a Medical Record Transfer Form available at any Pines Health Center
2) Relax, we’ll request your health information and transfer it into our system
3) We’ll call to schedule your first appointment with your new Pines provider once your health information arrives.
